This policy explains how personal data of people using the website aleksandrauzar.pl and contacting via the contact form is processed.

In connection with the use of the contact form we process the data you provide voluntarily:

• name,
• e-mail address,
• phone number (if provided),
• message content and the selected type of service.

We do not collect special-category data (e.g. health data) via the form — please do not include sensitive information in the message. Health-related matters are discussed only in secure conditions during a consultation.

• responding to your enquiry and getting back to you — on the basis of your consent (Art. 6(1)(a) GDPR) and to take steps prior to entering into a contract (Art. 6(1)(b) GDPR),
• possible establishment, exercise or defence of legal claims — on the basis of the controller's legitimate interest (Art. 6(1)(f) GDPR).

Data from the form is stored for the time necessary to respond to and handle your enquiry, and then until any claims become time-barred or until consent is withdrawn.

5. Data recipients

Data may be entrusted to trusted providers supporting the operation of the website and message handling, only to the extent necessary:

• the website hosting provider,
• the contact-form service provider (Web3Forms) — which forwards the message to the controller's e-mail inbox,
• the e-mail service provider.

Data is neither sold nor used for marketing profiling.

You have the right to: access your data, rectify it, erase it, restrict processing, object, data portability, and withdraw consent at any time (without affecting the lawfulness of processing before withdrawal). You may also lodge a complaint with the President of the Personal Data Protection Office (UODO) (ul. Stawki 2, 00-193 Warsaw, Poland).

The website is informational and does not use tracking cookies or profiling tools. Only technical elements necessary for the correct display of the site may be used (including Google Fonts). We do not carry out advertising tracking of users.
